It could be the modem, wiring or provider. Typically it is either the wiring or the provider.

Not likely to be a faulty modem… they tend to work or not work.
Situations where I’ve got low dial-up speeds:
Poor quality phone line (the line would get water in every time it rained and you could hear hissing through the handset).
Poor quality extension lead for the modem
Modem cable coiled up or wound round a power lead (My compaq laptop will only get 33.6Kb max when it’s modem cable is coiled up)
Pay-per-use internet suppliers sometimes drop the speed to ‘encourage’ you to stay on longer… but not too far or you fins someone else.
Try checking the dial-up line for hiss or crackle… dial ’9′ from the extension you use for the modem and listen… it should be quiet.
Try a different modem / PC see if that works the same or better.

Its more then likely your internet provider or your phone lines both could be your issue. I have dial up too and my issue actually turned out to be the phone company that has the wires outside my house are so old that they can’t transfer speeds any faster.
